NA form 13075 is the National Archives and Records Administration form the full name of which is Questionnaire About Military Service. A veteran may be asked to complete this form as a response to the initial inquiry form the administration because of their inability to have located the requested data or it got destroyed in the July 1973 fire that occurred at the National Personnel Records Center.

The information the form asks is supposed to help reconstruct some of the basic data on the military service records or enable the National Personnel Records Center to make use of available alternate sources of information.
When is the NA Form 13075 due?
The form should be filed only upon the request of the National Personnel Records Center along with any other forms that may have sent for completion. The Records Center requests all the claimant to fill out and return the form within 30 days after you received the request letter. Otherwise, they have enough grounds to stop the inquiry procedure.
